This is a sample lesson plan template that I use for Guided Reading plans. It is very easy to use. Simply print & make copies. Fill in the weekly information for each group & check off the skills that you want to cover that week.

I love this format because it quick & easy! I am in the process of creating quick & easy guided reading games, activities, worksheets, and more!
